If you wish to apply for an individual policy through BCBSM, please print the application and fax it (231-941-4085) or mail to our office. The BCBSM website is experiencing periodic errors which may result in your policy not being issued correctly.  A paper application submitted through our office will eliminate any potential errors or problems.

Please note effective dates of coverage are for the 1^st and 15^th of the month only, unless there is a group conversion.  Effective dates are 15 days after our office receives your application.  If you have any questions or are not able to download the application please contact our office (231-941-0450).  We are here to guide you through the process.

Assurant policies can be applied for online - without problems.
