If you wish to apply for an individual policy through BCBSM, please print the application and fax (231.941.4085) or mail it to our office. The BCBSM website is experiencing periodic errors which might result in your policy being issued incorrectly. A paper application submitted through our office will eliminate any potential errors or problems.
Please note effective dates of coverage are for the 1st and 15th of the month only, unless there is a group conversion. Effective dates are 15 days after our office receives your application.
